Job Description

POSITION SUMMARY :

Under the direction of the Integrated Behavioral Health (IBH) leadership team and in collaboration with the Health Equity Accelerator, the Population-based care IBH clinician will address the intersection of behavioral health and medical conditions across the lifespan. As a member of the Adult Primary Care, Child/Adolescent Primary Care, and Pediatric Medical Specialty teams, this clinician will work closely with primary care providers (PCPs) and medical specialists. The clinician will respond to referrals from PCPs and medical specialists and will utilize registry data to provide proactive outreach to patients at high risk for developing behavioral health conditions, including depression and anxiety. This clinician will also facilitate individual and group interventions for patients to improve their self-management of common co-occurring chronic medical conditions such as diabetes and hypertension, which can exacerbate behavioral health conditions. This position is currently a hybrid of telemedicine and in-person, subject to change based on hospital expectations and IBH program needs. The clinician will work closely within a multidisciplinary team of clinicians, psychologists, and psychiatrists in Primary Care clinics and embedded Medical Specialty practices.

ESSENTIAL RESPONSIBILITIES / DUTIES:

CLINICAL RESPONSIBILITES:

  • Clinician will conduct psychosocial assessments, crisis interventions as needed, formulate behavioral health diagnoses, and case conceptualizations, develop and implement effective short-term treatment plans, facilitate individual and group interventions, and collaborate with Primary Care Providers and Medical Specialists.
  • Clinician will adhere to trauma-informed, evidence-based principles in providing clinical care to patients and families across the lifespan.
  • Clinician will present cases in team meetings with a consulting Psychiatrist and Psychologist and communicate team recommendations back to Primary Care and Medical Specialty clinical teams.
  • Clinician will monitor patient outcomes using evidence-based tools (e.g. PHQ-9/PHQ-A; PSC-17) and adjust treatment as needed.

OTHER RESPONSIBILITES:

  • Attendance at IBH and Psychiatry meetings and in-service education offerings through the Department of Psychiatry
  • Attendance at General Internal Medicine, Pediatric/Adolescent Primary Care, and Medical Specialty Practice meetings under the direction of supervisor
  • Obtaining continuing education as necessary to maintain licensure
  • Follows established hospital infection control and safety procedures.
  • Maintains patient confidentiality according to hospital policy. Practices professional discretion when determining the inclusion of any patient information into medical record or department clinical record notes.
  • Documents each patient encounter in the medical record or the Department clinical record concurrently according to Department policies
  • Utilizes hospital's behavioral standards as the basis of decision-making and facilitating the hospital's mission.
  • Perform other duties as requested and or as necessary (e.g. coverage of IBH “warm-hand off” same-day clinical support, at the discretion of IBH clinical leadership).

  • Implements BMC protocol when suspecting domestic violence, and abuse or neglect of a vulnerable individual (e.g., children, older adults, those with medical or psychiatric disability).  
  • Maintains patient confidentiality
  • Utilizes hospital values as basis for decision making and to facilitate hospital mission
  • Follows established hospital infection control and safety procedures
  • Models the ability to set professional boundaries
  • Performs other duties as assigned and or as necessary.
  • Must adhere to all of BMC’s RESPECT behavioral standards.

JOB REQUIREMENTS

EDUCATION:

Requires: Master’s Degree from an accredited school of Social Work or school of Mental Health Counseling or related field as determined by the Board of Registration of Allied Mental Health and Human Services Professions or National Certified Counselor (NCC) preparing for licensure as an LMHC.

Preferred: Master’s Degree in Public Health (MPH) from an accredited school or a related field such as Population Health, Health Policy, Community Health, or Healthcare Administration, with a focus on behavioral health, population health, or health equity.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

Requires a Massachusetts license as a Licensed Certified Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or Licensed Independent Clinical Social Worker (LICSW). For LCSW, can be obtained within 6 months of employment. For Pre-LMHC, can be National Certified Counselor (NCC) preparing for licensure as an LMHC or Masters in Mental Health Counseling, or related field as determined by the Board of Registration of Allied Mental Health and Human Services Professions

EXPERIENCE:

  • Previous experience in Integrated Behavioral Health or Medical Setting preferred.
  • Interest in chronic medical conditions and culturally responsive holistic approaches to health and wellbeing for patients across the lifespan preferred.
  • Experience working clinically across the lifespan, strongly preferred.
  • Experience working with caregivers is a plus
  • Experience providing individual and group therapeutic interventions is preferred.

KNOWLEDGE AND SKILLS

  • Advanced interpersonal skills necessary to interact effectively with various healthcare professionals, to develop and maintain effective therapeutic relationships with patients and families
  • Cultural sensitivity and responsiveness humility with patients from diverse backgrounds and identities.
  • Bicultural/Bilingual competency is preferred (e.g., ability to conduct clinical assessment and intervention in a language other than English)
  • Requires knowledge of Social Work / Mental Health Counseling processes, as well as familiarity with community resources, agencies, and social determinants of health related to wellness, illness, and disability.
  • Ability to work in a multidisciplinary team.
  • Computer literacy with ability to use standard word processing, spreadsheet programs and e-mail (i.e. MS Word, Excel, Outlook) and web browsers.
  • Must be comfortable in a fast paced and dynamic work environment
  • Creativity, sense of humor, and flexibility is highly valued
